Special instructions (allergies, medications, epilepsy, deafness, etc.)

Immunization Requirements
All vaccinations** must be administered by a certified veterinarian or
vet tech, and verified as such.

Dogs must be immunized against:
Rabies
DHPP (distemper, hepatitis, parainfluenza, parvovirus)
Bordetella
Cats must be immunized against:
Rabies
FVRCP (panleukopenia or distemper, feline rhinotracheitis, calicivirus and pneumonitis-chlamydia)
** Any other variations must be verified by a certified veterinarian.

Things to Remember:
Make reservations early!!! Especially during seasonal events.
Use permanent marker or something to label your pet's items.
Let your emergency contact person be aware of your itinerary, pet's needs, restrictions and other important information.
Make sure all immunizations are current and on file with our staff.
If your pet is not used to your being away, please bring in a towel, tee shirt etc. (something with your scent on it)
